The modern jaw frequently lacks room for them, so they erupt at an angle, partially emerge, or remain buried in bone. A tooth in any of those positions is described as impacted, and impaction causes most wisdom tooth problems.
The usual reasons for removal are recurrent infection of the gum flap over a partially erupted tooth, decay in the wisdom tooth or the molar in front of it, cyst formation around an impacted tooth, damage to the adjacent tooth’s root, or persistent pain and swelling.

A panoramic X-ray shows root position and the relationship to the inferior alveolar nerve in the lower jaw and to the sinus above the upper teeth. That relationship determines the surgical plan and the risks to discuss.

A fully erupted wisdom tooth can often be removed straightforwardly. An impacted tooth generally requires a surgical approach, sometimes with removal of a small amount of bone and sectioning of the tooth.
Where roots sit very close to the nerve, or the case is otherwise complex, we refer to an oral and maxillofacial surgeon. Knowing when to refer is part of doing this properly.
