TMJ problems are frequently multifactorial — the joint, the muscles, the bite, clenching, stress and posture can all contribute, often at once. That is why management usually starts conservatively and why anyone promising a single definitive fix should be treated with caution.
The temporomandibular joints connect the lower jaw to the skull just in front of each ear. Temporomandibular disorder is an umbrella term for problems affecting those joints, the muscles that move them, or both. Symptoms include jaw pain, clicking or grating, restricted opening, headaches and facial muscle tenderness.
Most cases are muscular rather than structural, and most improve with conservative management. Clenching and grinding, often at night and often unnoticed, are among the most common contributing factors, and they also wear the teeth down and crack them over time.
Assessment is worthwhile where jaw symptoms are persistent or affecting daily life.
Management begins with the least invasive measures and escalates only where necessary.
We ask about pattern, timing and triggers, which often points to the dominant factor — night-time clenching presents very differently from a joint problem.
We examine the joints, muscles, range of movement and bite, and look for wear patterns on the teeth.
Clenching, grinding, stress, posture and habits such as nail biting or chewing gum all contribute. Some are addressed with awareness alone.
Soft diet during flare-ups, jaw rest, heat, gentle exercises and avoiding wide opening resolve a substantial proportion of cases without any appliance.
A custom occlusal splint worn at night protects the teeth from grinding forces and can reduce muscle activity. It is a management tool rather than a cure. Occlusal splints start from $770.
We review response over time. Where symptoms persist or a structural joint problem is suspected, referral to a specialist or for imaging is appropriate.
TMJ management is generally conservative, but expectations need setting realistically.
Possible Risks:
Recovery and Aftercare:
A splint manages symptoms and protects the teeth from grinding forces. It does not cure a TMJ disorder, and anyone presenting it as a cure is overstating what it does.
Many patients find it substantially reduces pain and morning headaches, which is a worthwhile outcome in itself.
Usually not, and irreversible bite adjustment should be approached with real caution. Conservative, reversible measures resolve most cases.
We would not recommend permanently altering your teeth on the basis of jaw symptoms without a very clear rationale.
Clicking without pain or restricted opening is common and often does not require treatment at all. Clicking accompanied by pain, locking or limited opening warrants assessment.
Frequently, yes. Clenching and grinding increase markedly with stress, and many patients notice symptoms tracking their workload.
That does not make the pain any less real, and it does not mean nothing can be done — but it does mean management often involves more than dentistry alone.
